Insurance Waiver Request
If you want to request a waiver for any of the insurance mandates, contact the RI Division of Purchases with your request. Depending on the waiver being sought and if it includes any type of reduction to recommended cyber/IT insurance coverage, Purchases may engage with ETSS/CISO as needed for evaluation/determination.
Please complete the Insurance Waiver Request form and upload it using the webform below to submit your request. Once submitted, it will be forwarded to the RI Division of Purchases, and you will automatically receive a confirmation copy for your records.
